We are excited to announce that we are 1 week away from reaching our target enrollment goal! We have collaborated with top home care clinicians and worked with incredible patients. With countless nursing and rehabilitation visits for our patients, we are proud to be engaging with individuals and families across Manhattan. Through personal interviews with patients in their homes, we are able to connect with our patients to hear their stories and engage them in improving our program for future patients. While we are almost finished with the data collection, the program will continue to flourish independently, as trained clinicians bring specialized cardiac care to patients beyond our pilot period.
The vision for this project was to make a larger impact for patients and families, and we aim to achieve this goal through nationwide dissemination. With a manuscript under review with a leading journal in cardiac rehabilitation, and another submission for publication on the way, our work has the potential to be disseminated to interdisciplinary cardiac rehabilitation teams and professionals across the country. Through our relationship with the CMS Million Hearts Initiative, alternate models of cardiac rehab delivery are being discussed through the lenses of healthcare policy reform. Finally, at the American Association of Cardiovascular and Pulmonary Rehabilitation Annual Conference in the fall, the Home Heart Health program for home-based cardiac rehab will be presented alongside other innovative alternate models of care, by a panel leaders in the fields of cardiovascular and pulmonary medicine, research, clinical practice, and innovation. These dissemination platforms pave the way for future studies and healthcare policy changes down the road.
