A PATIENT- DRIVEN HOME- BASED CARDIAC REHABILITATION MODEL
With the suboptimal rates of participation and adherence to outpatient cardiac rehabilitation, there is a need for alternate strategies and delivery settings for cardiac rehabilitation for this vulnerable patient population. Home-based cardiac rehabilitation programs are a potential method for improving clinical outcomes in a cost-effective manner, in addition to increasing attendance and adherence to outpatient cardiac rehabilitation. In order for home-based cardiac rehabilitation to be integrated into home care agencies and offered as an alternate or supplementary service to outpatient cardiac rehabilitation, there is a need for a standardized, evidence-based model for home-based cardiac rehabilitation.
Our plan is to collaborate with patients, home care clinicians, physicians, cardiac rehabilitation experts, and other key stakeholders to design, evaluate, and implement a patient-driven home-based cardiac rehabilitation model.
